Sabah Maternity hospital documents

While we were in Sabah Maternity hospital last week, we found out that a series of documents were required, that have to be kept ready for all sorts of activites from admission to visit to discharge to birth certificate for the new born. There is a handy list of these documents printed and pasted on the walls of Sabah Maternity hospital wards. Took a pic of the printout so apologies for the poor quality. I have converted this into a PDF document for easy download and printing.

One important thing about marriage certificates, Sabah hospital will only accept Arabic version of marriage certificates. If you dont have one then go to your embassy and get an affidavit certifying your marriage certificate, which is in English/Arabic [bi-lingual], and then you will have to get this affidavit attested from the Ministry of External Affairs Kuwait. Ministry of External Affairs building is located on Route 80 just before City Center Shuwaikh and next to Red Cross building.

    Also it makes me greedy. Will be of great help if you can post the procedure for getting various documents related to the baby. Where all one has to do around and what all should be collected before we proceed to get the residency and civil id ?

    Dear DA, Just wanted to know if the original marriage affidavit is required for admitting ur wife at Sabah hospital? Is it ok to submit just the copy of the affidavit.

    The reason I am asking is because I have lost the original and currently only have a copy. While opening the file at the clinic the copy was enough.

    Yes the original is required. The hospital will not accept any documents without originals. Also make sure the names on Marriage certificate are exactly the names as on Civil ID especially Arabic ones.
